Dartmouth Health Connect, a primary care medical practice for Dartmouth employees, “appears to be exceeding expectations in its effectiveness,” writes the Valley News in an article about the practice, which opened its doors in March 2012.

Unlike traditional primary care practices, Dartmouth Health Connect uses a team-centered approach for patient care and each patient has a health coach, explains the Valley News.

According to the article, Dartmouth Health Connect now treats 1,200 individuals. “It’s going great,” Melissa Miner, director of health promotion and wellness at Dartmouth, tells the Valley News. “I think the employees have really spoken for how the model is working, through enrollment and through the feedback.”
